Note that once a plan change is made, you will be locked into that plan for two years. After filling the form, please send the signed original copy to NADA.
Our church cannot afford to enroll in the plan every employee who works at least 20 hours per week (15 hours per week in Saskatchewan). Is there a way to adjust the eligibility requirements?

If employers want to set eligibility requirements by increasing the minimum number of hours worked per week that defines a full-time employee, they can do so. In this case, the employer should have a written policy related to offering insurance and set the criteria set for eligibility as a full-time employee. Note that if this happens, every employee who qualifies under the newly established hours per week criteria will still need to be enrolled in the plan.

I am enrolled in a plan with my church, but I would like to add some optional benefits to my plan. Can I do it?
Individual plan members have a number of optional benefits. These include: Preferred Life Insurance, Preferred Spouse Life Insurance and Voluntary Death and Accident Distribution Insurance. Your plan does not cover dental or vision care unless it is already provided. For more information about adding optional benefits to your plan, please contact nada.
I was enrolled in the plan but now my spouse has coverage, and I no longer wish to receive benefits through C. Can I withdraw from the plan?

You may not drop the plan entirely, but you may drop advanced health care and/or dental care (if applicable) if you are covered by your spouse. If so, you need to file an employee application for insurance, stating that this is a change in coverage with an effective date. Under the “Coverage Requested” heading, check “I decline to participate” and the appropriate box. Once the form is complete, please send it to NADA with the original record to forward to CannaBenefits.
I am married or have children. How do I change my coverage to include my new family member?

When you receive a salary change, you must ensure that your employer notifies CanBenefits of the change. Because payments for life insurance and long-term disability payments are related, it is important for your employer to keep this information up to date. If you are required to collect insurance in one of these areas, the amount paid will be based on the last salary on file with the insurance company, even if your original salary is greater than the amount on file.
